AND implementation

Answer the questions below — only the options that make sense for your situation will appear as you go.

To start, tell us more about where you work.

I work at or lead a community-serving organization (e.g., non-profit, foundation, government, co-op, collective) I am a researcher, evaluator, or implementation scientist I work doing both, between, or outside the above options

1. What best describes why you're hoping to connect with Maya and Jennica at AND implementation?

Want a training for myself or my team Want them to create something for me (e.g., a survey, interview guide, or logic model) Want them to complete part of a project for me or my team (e.g., do interviews, create an evaluation framework, analyze data) Want to collaborate with them on a project (e.g., sub-contract, partner) Want to take their arts-based course Want their help thinking through a problem or idea I am working on Want help improving our data system (e.g., what data do we have? Where are there gaps? How can we use our data better?) Want help with work associated with my Masters or PhD thesis Something else (nothing above is quite right)

2. What best describes how you want to work together?

I would like to work independently, with some meetings to help me along the way I want us to work together throughout the process I would like them to do the work independently

3. What do you hope comes out of the work together?

A project is completed A deliverable is given to me (a framework, tool, report, or primary data) At least one training or workshop is completed Some meetings have happened that helped me move towards my goal Other
